If you are a new patient, please check with your insurance plan to determine if you need an insurance referral from your primary care physician to be seen at Bella Dermatology. Also, please download and fill-out the new patient forms and bring them with you to your first visit. Finally, please bring your insurance card, driver’s license, and medication list to your first visit.

SELF-PAY CLIENTS AND PATIENTS WITH OUT OF NETWORK INSURANCE BENEFITS:

Bella Dermatology is not a provider for Cigna, traditional Medicare, Medicare through Prominence Health Plan, nor United Healthcare. We are accepting self-pay patients and patients with out-of-network insurance benefits. If we are out-of-network providers, you are responsible to pay your bill directly to Bella Dermatology at the time of your visit. We will then provide you with the necessary paperwork for you to bill your own insurance company.
